This Surrogate Applicant Acknowledgement, Consent, and Release (the “Acknowledgement”) is entered into between True Match Surrogacy Ltd. (the "Agency"), a business incorporated under the laws of the Province of British Columbia in Canada, with a mailing address of PO Box 21077, 2271 Harvey Avenue, Kelowna, BC, V1Y 9N8 and you, the applicant.

Identity Verification

Please provide scanned or photographed copies of two valid government issued photo ID:

1) Passport, or

2) Driver's License (both sides), or

3) Medical / Health Services Card (both sides)


Proof of Citizenship/Residency

Please provide scanned or photographed copies of:

1) Birth Certificate, or

2) Citizenship Card/Certificate, or

3) Permanent Resident Card


1. Purpose and Nature of This Document

This Acknowledgement governs the Applicant’s voluntary application to participate in the Agency’s screening, education, and information process. This Acknowledgement is not a surrogacy agreement, does not create an obligation to proceed with surrogacy, and does not guarantee matching with intended parents.

2. Voluntary Participation and Eligibility

The Applicant confirms that participation is entirely voluntary; the Applicant is at least 21 years of age; and no person has coerced, pressured, or induced the Applicant to apply. The Applicant may withdraw from the application process at any time by providing written notice to the Agency.

3. Accuracy of Information and Continuing Disclosure

The Applicant represents that all information provided is true, accurate, and complete. The Applicant agrees to promptly notify the Agency in writing of any material change, including changes to health status, medications or treatments, marital/relationship status, criminal charges or convictions, and contact information.

4. No Legal or Medical Advice

The Applicant acknowledges that the Agency does not provide legal advice or medical advice. No Agency representative acts as the Applicant’s legal counsel or health-care provider. The Applicant is encouraged to obtain independent legal advice at any stage of the process.

5. No Guarantee and No Control of Third Parties

The Applicant understands that the Agency makes no guarantees regarding matching, timelines, pregnancy outcomes, or parentage determinations. The Agency does not control the conduct of intended parents, fertility clinics, counsellors, lawyers, or other third parties.

6. Consent to Collection, Use, and Disclosure of Personal Information

The Applicant consents to the Agency collecting, using, storing, and disclosing personal information, including photographs and non-diagnostic screening information, solely for purposes related to screening and education, potential matching, and coordination with intended parents, fertility clinics, counsellors, and legal professionals. This consent is provided in accordance with the Personal Information Protection Act (British Columbia) and applicable federal privacy legislation. Withdrawal of consent may result in termination of participation.

7. Privacy and Data Handling

The Applicant acknowledges that limited technical data (including email address and IP address) may be recorded for security, audit, and compliance purposes. The Agency will implement reasonable administrative, technical, and physical safeguards to protect personal information and will retain personal information only as required for legal, regulatory, or operational purposes.

8. Release and Limitation of Liability

The Applicant shall notify the Agency in writing immediately upon becoming aware of any issue, dispute, complaint, concern, or dissatisfaction relating to the Agency’s services, operations, policies, or personnel. Such notice is intended to allow timely review and resolution and does not limit any legal rights or remedies available to the Applicant.

9. Mandatory Written Notice of Issues and Concerns

The Applicant shall notify the Agency in writing immediately upon becoming aware of any issue, dispute, complaint, concern, or dissatisfaction relating to the Agency’s services, operations, policies, or personnel. Such notice is intended to allow timely review and resolution and does not limit any legal rights or remedies available to the Applicant.

10. Applicant Portal and Code of Conduct

Any agency portal or communication platform shall be used respectfully and lawfully. Harassment, abuse, impersonation, or misuse may result in suspension or termination of access.

11. Withdrawal from Participation

The Applicant agrees to notify the Agency promptly and in writing if the Applicant no longer wishes or is no longer able to participate in the application process.

12. Notices

Any notice under this Acknowledgement must be in writing and delivered by email. Notices to the Agency shall be delivered to hello@truematchsurrogacy.ca. Notices to the Applicant shall be delivered to the email address provided above, unless updated in writing.

13. Governing Law

This Acknowledgement shall be governed by the laws of the Province of British Columbia and the federal laws of Canada applicable therein.
