The Intended Parent Marketing, Promotion and Consent Agreement (the “Agreement”) is between True Match Surrogacy Ltd. (the "Agency") or ("TMS"), a business incorporated under the laws of the Province of British Columbia, with a mailing address of PO Box 21077, 2271 Harvey Avenue, Kelowna, BC, V1Y 9N8 and the intended parent(s) named below.

Collectively, the above named person(s) are the “Intended Parent(s)” or “Client(s)”.

Collectively, the Intended Parent(s) and True Match Surrogacy Ltd. are the “Parties” and individually a “Party”.

Purpose of Collection

True Match Surrogacy Ltd. (“TMS”, “we”, “our”, or “the Agency”) may collect personal information from intended parents who wish to share aspects of their surrogacy journey, personal story, or family profile.

The purpose of collecting this information is to:

- Introduce intended parents to potential gestational surrogates

- Share personal journeys and experiences relating to family building

- Promote awareness of altruistic surrogacy in Canada

- Support outreach and marketing initiatives of True Match Surrogacy Ltd.

Information provided may be used in:

- Agency website profiles

- Surrogate-facing informational pages

- Educational materials

- Social media posts

- Digital advertising

- Print or promotional materials

- Presentations, webinars, or informational campaigns

Participation is voluntary.

Information That May Be Collected

The information requested may include, but is not limited to:

- First names or preferred names

- City or region of residence

- Personal background and family story

- Photographs or videos

- Lifestyle information or interests

- Reasons for pursuing surrogacy

- Messages intended for prospective surrogates

Participants should avoid including sensitive information they do not wish to be publicly disclosed.

Consent to Use and Disclosure

By submitting this form and signing below, the Intended Parent(s):

1) Authorize True Match Surrogacy Ltd. to use, reproduce, publish, adapt, and distribute the information provided for promotional, informational, and outreach purposes.

2) Understand that this information may be made publicly available through online platforms including websites, social media channels, and digital advertising.

3) Grant TMS the right to edit, format, shorten, or adapt the submitted content for clarity, style, or presentation purposes, provided the meaning of the content is not materially altered.

4) Understand that the content may be viewed by the general public, including individuals outside Canada.

Optional Anonymity

Participants may request one of the following privacy options:

- Use first names only
- Use initials or pseudonyms
- Use no names (anonymous profile)
- Full names permitted

True Match Surrogacy Ltd. will make reasonable efforts to honour the selected privacy preference.

Release and Waiver

The Intended Parent(s):

- acknowledge that participation is voluntary

- release and hold harmless True Match Surrogacy Ltd., its directors, employees, contractors, and affiliates from any claims arising from the lawful use of the submitted information in accordance with this consent

This includes claims relating to:

- reputation

- publicity

- privacy

- emotional distress arising from publication of the content

except where such use results from gross negligence or willful misconduct by the Agency.

No Guarantee of Surrogate Interest

Submission of information and participation in promotional materials does not guarantee:

- interest from potential surrogates

- matching with a surrogate

- participation in any specific program or process.

True Match Surrogacy Ltd. provides facilitation services and cannot guarantee outcomes.

Withdrawal of Consent

Participants may withdraw their consent to use their information in future marketing materials by submitting a written request to:

Upon receipt of a withdrawal request, TMS will make reasonable efforts to remove the content from future use. However, previously published materials (including archived or shared social media content) may not be fully retractable.

Data Protection

Personal information will be handled in accordance with:

- the Personal Information Protection and Electronic Documents Act (PIPEDA)

- the British Columbia Personal Information Protection Act (PIPA)

- the Agency’s Privacy Policy

Acknowledgement

By signing below, the Intended Parent(s) confirm that:

- they have read and understood this consent form

- they voluntarily provide the submitted information

- they authorize True Match Surrogacy Ltd. to use the information as described above
