I understand that individuals on the waitlist who have not paid the agency fee are not guaranteed access to services within any specific timeframe and may remain on the waitlist indefinitely, subject to availability and the agency’s sole discretion.

By submitting this form, I expressly consent to the collection, use, disclosure, and secure retention of my personal information by True Match Surrogacy Ltd. for the purpose of assessing my eligibility as an intended parent, responding to my inquiry, facilitating potential surrogacy matching services, and for related administrative and communication purposes, in accordance with the Privacy Policy.
